Conditions of Admission

Issued to every patient on arrival

The following was printed on the reverse of the intake form and signed by the patient before a bed was allocated.


You are here of your own accord. You may say so at any time and it will be written down.

You will be observed at night. This is not a punishment and it is not optional. The observation window in your door is not locked and may be covered on request, except during a scheduled observation.

You will be asked each morning what you remember. Answer plainly. There is no answer that will be held against you, and no answer that will be disbelieved.

If you are moved to another ward you will be told the reason. If you are not told, ask, and it will be written down that you asked.

Meals are at seven, noon and six. Lights out is at ten. The corridor lamps remain lit through the night and are not turned off for any reason.

You may leave. Say so to any member of staff, at any hour, and the discharge procedure will begin the same day.

Discharge requires the attending physician to certify that you are as you were when you arrived.
